VON’s Passport Program
VON’s Passport Program provides personalized support for adults with developmental disabilities, 18 years of age and older. The program is intended to develop the skills and independence of those supported and enhance community participation. It may also provide meaningful respite to family caregivers. The program is funded by the Ministry of Community and Social Services.
Adults with disabilities, potentially with the assistance of their family members, may apply for the program. If you do not yet receive government funding for Passport, we’ll help you navigate the steps to apply with Developmental Services Ontario (DSO). If you have access to Passport funding, VON can assist you to coordinate your service and develop a personalized plan to fit your needs.
Special Services at Home (SSAH)
SSAH offers respite and support on an individualized basis to help primary caregivers by providing temporary relief support. SSAH is for people of all ages who have a developmental disability and for children under the age of 18 who have a physical disability. Self-referrals are encouraged. Respite Coordinators are assigned to assist families to apply for the Special Services at Home program and to plan their respite services.
